Hospice Care for Healthcare Providers
A Trusted Partner in Compassionate, Physician-Led Hospice Care in Arizona
Hospice Care For Healthcare Providers
At Neighborhood Hospice, we understand that referring a patient to hospice is one of the most important decisions a healthcare provider can make. Our goal is to become a trusted extension of your care team by providing compassionate, physician - led hospice services built on clinical excellence, education, and collaborations.
We are committed to making the referral process seamless while ensuring patients and families receive the personalized care, dignity, and support they deserve.
Why Healthcare Professionals Choose
Neighborhood Hospice
Physician-Led Care
Every patient referred to Neighborhood Hospice is personally evaluated by Dr. A. Collins Villamor, Medical Director.
Unlike many hospice organizations, Dr. Villamore remains actively involved throughout each patient's care, collaborating with the care team and referring providers to ensure compassionate, personalized treatment that reflects the patient's goals and medical needs.
Education Before Enrollment
We believe hospice begins with education.
Patients and families deserve honest conversations, compassionate guidance, and the opportunity to fully understand their options before making decisions about hospice care.
Our approach is never rushed or pressured. We take the time to answer questions, explain the hospice benefit, and ensure families feel informed and supported.
Collaborative Care
Neighborhood Hospice values strong partnerships with physicians, specialists, hospitals, case managers, and facility staff.
We are committed to:
-
Prompt communication
-
Timely patient evaluations
-
Collaborative care planning
-
Ongoing physician involvement
-
Respect for your clinical relationship with the patient
Medicare Eligibility
Medicare covers hospice care for eligible patients who:
-
Are enrolled in Medicare Part A
-
Have a terminal illness certified by a physician with a life expectancy of six months or less if the illness follows its expected course
-
Choose to receive hospice care focused on comfort and quality of life rather than curative treatment
Hospice eligibility is based on a patient's medical condition and may be reviewed throughout their care. Patients who live longer than six months may continue receiving hospice services if they continue to meet Medicare's eligibility criteria.
Physician Evaluation
At Neighborhood Hospice, every patient is personally evaluated by our Medical Director, Dr. A. Collins Villamor, to ensure hospice is the appropriate level of care. We work closely with referring physicians and healthcare teams to provide a smooth transition while honoring the patient's goals, wishes, and plan of care.
Our commitment is to partner with referring providers to deliver timely, compassionate, physician-led care that supports both patients and their families.
